Ruby Falls in Tennessee, this is the biggest underground waterfall in the country that was discovered by accident.  According to the tour guide, they aren't sure where this water is actually coming from..?  It is incredible though, you take an elevator like a thousand feet down into this cave, then walk through the cave for about half an hour and this is at the end.

He was drilling a shaft for an elevator down into the mountain so he can take tours through to the caves and on his way down they discovered a small void in the rock about 18 inches high and 5 feet wide that he and two buddies decided to explore. They had to crawl their way through about half way until they finally got to a point where they could…

Ruby Falls at Lookout Mountain TN. We stopped here on our honeymoon and absoultely loved it. There is a tight elevator ride to the bottom and a long cave to walk through to get the falls. Might be tough for someone who is afraid of tight places or caves.
